All the Photocopies must have your thumb impression and Sign (back side)All the original Certificates must be coated.
1-Original SSC Certificate
2-Original HSSC Certificate
3-NOC/Migration Certificate
4-Original Domicile
5-Character/Provisional Certificate
6-Mdcat Mark Sheet
7-CNIC(1 copy each) of Father and Candidate
8- 2 Copies of FSC Certificate for Verification
9- Receipt of Schduled Fee Deposit from BISE for Verification. ( Fee = 2X ) for HSSC only.
10-One set of Attested Copies (SSC Certificate, HSSC Certificate , 2 photograph ) for UHS Registration.
11- One set of Attested Copies (SSC Certificate, HSSC Certificate , 3 photograph ) for PMDC Registration.
12-Attested 3 Sets of All Documents + 12 Photographs (6 ID Card + 6 Passport size)
13-Surety Bond on Rs.100 Stamp Attested duly by 1st class Magistrate
14- Undertaking on Rs.100 Stamp Attested duly by 1st class Magistrate
15-Vaccination Certificate for Hepatitis B, Tetanus and enteric group of Fever.
16-Attach Printed/Recieved Call Letter
